According to Howard, that's the first pen he's missed in matches.
"I'm very disappointed to miss it, I've taken penalties throughout my career and that's the first one I've ever missed so hopefully it won't happen again," said Howard.
I tried Googling to see if I could find evidence of one he missed, but couldn't.

Snowball Pen didn't look THAT bad, either
I've seen worse. Was hit hard but at a good height for the 'keeper, who was about 6 foot off his line when he saved it.
