By those parameters only, Wokingham Town...just...lived in Reading from 0 to 2-and-a-half, then Wokingham until 7.

Forres Mechanics my first team as 5yr old. 
My first 3 addresses would read as follows:
1. Dagenham & Redbridge, League 2
2. Great Yarmouth of the Ridgeons Football League Premier Division (Norwich nearest league team I guess)
3. Reading FC

Born in Whitley Wood so that's easy. However moved to the North of Scotland almost immediately, so years 1-6 would be Inverness (but they were non-League then) or Aberdeen (?).
As has been established before I was very very close to SFers.

First 6 years of life would be Charlton or Dartford.
